Schedule Change Form

Please use this form to make schedule changes to summer 2020 and fall 2020.  This form is to convert classes from face-to-face meetings to either synchronous or asynchronous only.  For questions regarding this form, please contact Cheri Winter at 527-4520 or via email,

If your entire department will move to completely synchronous, please do not use the web form. Please contact Cheri directly on the best way to make these changes.   The web form is really intended for classes that will meet part of the time in a synchronous format with the remaining class content asynchronous or those sections that will be 100% asynchronous.  

Please complete one section per form (unless classes are combined).
